VA Report Finds Six Veterans Died After Delays

The Inspector General for the Department of Veterans Affairs released a report Tuesday that found six patients died after delays in care at the Phoenix V-A hospital. Investigators denitrified 28 instances of significant delays in clinical care and patient scheduling at the facility. But, they were not able to conclude the "absence of timely quality care caused the deaths of these veterans."
