MANSFIELD, OH - The Mansfield Police Department is investigating a two-vehicle crash that claimed the life of one man on Friday afternoon. 

According to police, the crash occurred when a black truck and a silver minivan struck each other head-on near the intersection of State Route 13 and Richland Shale Road. A 46-year-old man was pronounced dead at the scene. A 44-year-old female was transported to OhioHealth Mansfield Hospital and treated for injuries. Her condition is unknown at this time.

According to a Mansfield Police Crash Report, the 46-Year-Old man was identified as Trevor Tea, of Clyde, Ohio. The 44-year-old woman was identified as Cherrell Levenchuck, of Plymouth.

It is currrently unknown if drugs or alcohol were involved in the crash, or if either of the occupants were wearing safety belts. The crash is currently under investigation by Mansfield Police, pending further investigation by the Richland County Coroner's Office. The Mansfield Police Department was assisted by the Mansfield Post of the Ohio State Highway Patrol.
